Right at Home, a leader in in-home care for seniors and adult with disabilities, has partnered with PocketRN, a virtual nursing care platform, to provide care for dementia patients through the Guiding an Improved Dementia Experience (GUIDE) model from the Centers for Medicare & Medicaid Services (CMS).
The partnership will give Right at Home clients access to PocketRN's "virtual nurse for life" system, which matches patients and their caregivers with a nurse who provides round-the-clock education, coaching, monitoring and support. It will also enable Right at Home agencies to offer respite care services, a prominent feature of the GUIDE model.
"We couldn't be more thrilled to bring our revolutionary nurse-led care model to the millions of dementia patients and families who need it most," said Jenna Morgenstern-Gaines, CEO of PocketRN. "Nurses are hands-down the best clinicians to be the 'glue' to dementia patients and families throughout their journey — they've been doing so forever and it's high-time their work is valued by our system."
CMS launched the GUIDE model last year with the goal of reducing burdens for caregivers of people with dementia. The model includes reimbursing providers for care planning, management and coordination services, as well as respite care.
Right at Home is currently testing its partnership with PocketRN in a handful of markets and plans to expand it nationally by July.
"The GUIDE groups will not function without partners, but the participating organizations are the ones that have the contracts and the contracts obligate them to have partners that deliver the services," said Doug Robertson, senior director of government affairs at Right at Home.
